Who is eligible for hospice care?

Any person of any age who has been diagnosed with a terminal illness and given a prognosis of six months or less may be eligible for hospice care. Hospice is not limited to the elderly or to cancer patients; heart disease, COPD, dementia, neurological disorders, and many other life-limiting illnesses qualify. What types of hospice care does Choice Hospice offer?

Choice Hospice provides four levels of care: Routine Home Hospice Care, Respite Care, Continuous 24-Hour Home Care, and Short-Term Inpatient Care. Our team works with each family to determine the right level of care at every stage of the journey.
